Referrals to INTEGRATIVE can be made by your physician or specialist, nurse practitioner, osteopath, chiropractor, dentist or podiatrist.

INTEGRATIVE also provides DIRECT ACCESS PT (without a physician’s referral) for up to 30 calendar days after which a referral is required. We will send your Plan of Care to your Primary Care Provider or specialist to keep them informed of your therapy & progress and to help obtain your referral.

INTEGRATIVE is a participating provider for the following networks: Tricare Select and Prime, Anthem/Blue Cross & Blue Shield PPO and Federal, Aetna PPO, Cigna PPO, Optima PPO, and worker's compensation plans. Please remember that your individual copays, deductibles, and any uncovered expenses are your responsibility and these are collected at the time of your appointment. If you have questions regarding your insurance plan and its coverage please be sure to contact your insurance provider to verify or get approval if needed. INTEGRATIVE also welcomes our self-pay patients (Self-pay evaluation is $150, follow-up treatments are $120).
